Voice mail box that is not set up?

I have visual voicemail (Verizon Note 4). Worked great for a month or so. Now whenever someone calls, my phone tells them I have not set up my mailbox. I've gone into the app and verified that it is set up and ready.

What gives?

I think the trial expired. You'll need to click set up basic and go through the steps again

Nope. Still not working. Paying for the premium service, greeting is recorded... but I cannot get voicemails from missed calls.

I would call the carrier and see what they say. Sounds like their issue.
