Why can't I disable the swipe lock screen and use pin or password?

I'm on a rooted Verizon S4 which I recently upgraded to Android 5.0.1 (OF1 build). There is a strange issue occurring, where I cannot set a secure (Pin, password, etc.) lockscreen. If I attempt to use one, it accepts the changes and says "Secured with pin", but will keep the swipe (non secure) setting. It does not activate the pin lock whether I turn off the screen or let it time out.

Another oddity is if I go back into the settings to modify the pin, it will say the current pin i entered is invalid, even though I am 100% sure it is correct. I also cannot change the settings for "smart lock", or anything else that requires the pin, since the system believes anything i enter is incorrect.

Does anyone know what I could try in order to solve this? I attempted a fresh factory reset multiple times, which has been unsuccessful. Thanks!
