Why do my phone's screen switches off during the call?

When I make a call the screen directly switches off and so I can't end the call until I tell the person I called to hang and immediately after hanging up the screen turns on

Welcome to Android Central! Which phone? This is usually due to something blocking the proximity sensor, which is right by the front-facing camera. Do you have a poorly fitted screen protector or case on?

Which phone ? Does tapping the screen or hit power button wake it back up ?
