Why does my S4 suddenly think it's not charging?

S4 has started draining FAST after last update. Now at 15% I plug in to charge (wall charger). I seem to have 2 problems. 1) Draining faster than charging and 2) Settings show charging... then it seems to forget it's plugged in and says discharging. Unplug and plug power back in and it says charging again.
What should I be looking for?

Have you tried a different charger and USB lead?

Also, check the USB port for dirt and damage.

Happens with both wall chargers. Phone is not being moved while charging. If usb is dirty/damaged, why does it work for a while and then not?
What do I have to do to check/clean usb port?
