Why does my Samsung S4 mobile go off all of a sudden?

Samsung S4 mobile goes off all of a sudden.

Model Number : GT-I9500
Android Version: 5.0.1

Samsung S4 mobile goes off all of a sudden, when it has around 45 percent battery and the next time it comes up it shows 0 percent battery and then again goes off.

I tried removing the battery rubbing it a bit and then putting it back. The next time it boots it comes up showing some 20 percent to 25 percent battery instead of 45 percent.

Note: Battery percentage is not constant. Each time it goes off on different percentage. Most of the times it is below 50 percent.

How old is the battery? It may be time to replace it.
