Why is my Bluetooth crashing when turned on?

My wife and I both have OnePlus 3s and have both updated to Android Pie. My Bluetooth function is fine and works normally. Hers, on the other hand, crashes every time she attempts to turn it on or connect to a new or current device. We have cleared both the Bluetooth cache and storage and reset all connection settings. We have restarted the phone several times. The app works fine until the Bluetooth hardware is turned on. Any ideas? Thank you!

Just wiped the cache, but unfortunately same issue.

After you clear cache did you force close it?

My recommendation would be factory reset , something got corrupted .
I'm also surprised the one plus 3 got pie update , great job one plus
