Widgets missing from screens after restart FROM safe mode

After restarting FROM safe mode back into normal mode, my widgets are not showing back up on the screens. I have a clock widget on the home screen that is a preloaded widget (not third party) and when I power into safe mode, then back into normal mode, the widget; and all the other widgets I have on my screens, disappear. Even Command Center doesn't show back up. This is a pre installed widget.

I have not done a factory data reset yet but will try this. I know widgets are disabled when in safe mode, but they have shown back up when I rebooted to normal mode in kit kat.

Any reason why this would occur? Is this some issue with 5.1.1? Any idea of any resolution or time frame for resolution?

Safe mode is meant only to check if any third party applications are causing issues with the phone. I'd say the real answer is this: don't boot into safe mode unless you are trying to diagnose a problem.

This occurs probably because the apps that are associated with the widgets are disabled when you boot into safe mode, so the launcher decides that the app is no longer available and removes the widget.
 
I agree with doogald. There's a slim chance that something got stuck in the launcher cache after safe mode so you could try clearing it and then reboot your phone. But most likely you'll need to add all your widgets back.
 
One possible solution, if you really need to use safe mode often: switch to a third party launcher like Nova Prime or Action Launcher, which allows you to back up and restore your home screen layouts. You'd be able to restore the layout if you found the widgets missing. Because of a security design in Android, the launcher will display a message asking you to tap the widget location to reactivate it.
 
This is normal behavior for 5.1.1. I have booted into safe mode a few times to test some things, and each time I had to add my widgets back. As doogald said, you can use a launcher (I now use Nova) to avoid this. (Edited to mention I have the Turbo, not the Turbo 2, but I imagine it works the same).
