won't upgrade

I've tried upgrading from settings several times, but it doesn't work.
Tried once and it didn't work
Did a factory reset and tried again, still didn't work.
Did another factory reset and tried again, still didn't work.
It downloads the update and then appears to install it, but I get a "software update unsuccessful, no changes were made" message every time.

My first try was a failure. I removed the SD card, did a soft reset by removing the battery, and tried again. 2nd attempt worked fine.

The phone stopped recognizing my sd card also.
Verizon is sending a replacement.

That's a hard reset when you pull the battery, soft reset is when software restarts the phone. The key combo for restarting your phone if it locks is supposed to be a hard reset as well.
