How can I get the S Health App on my Note 4 permanently off of it?

The S health app, cannot remove it only disable it! it has found it's way to the lock screen and would like to know how to remove it from the lock screen and totally disable or remove it. Can someone please help?

Go to settings - device - lockscreen - show information - uncheck the pedometer box.

This app cannot be removed due to this app is pre-installed in your device from Samsung. The only download apps be able to remove. This is simple as happen quite long time ago.
