ringtone not working when receiving a call?

I noticed recently that when I receive a call the phone will not ring, only vibrate. I checked the sound settings and everything is turned up to the max volume. I also noticed that when using the volume buttons I can no longer go to silent or vibrate only, it just turns the slider all the way down without changing the mode. Can't figure out what happened. I'm running the stock 4.0.4 leak, not rooted.

When you choose a ring tone can you hear it? When adjust the volume on the ring tone can you hear that?

Yeah I can hear the ring tone play when in the sounds setting menu. Everything else works fine sound wise too, just not the incoming ringer. I thought I saw a topic once on here with similar problems, but can't find it if there was.

I've noticed that lately, as in over the weekend, my ring tone set for my wife sometimes does not work. Rather, the default ring tone plays. Minor glitch I'm sure....
