This basically means that your operating system has been corrupted. Your phone is essentially stuck in bootloader mode. You will probably have to use ODIN to go back to stock.

You are in download mode which is used to flash Samsung factory image. You can ask in the S4 section or Google on how to use Odin to restore factory image, but the phone might need an unlocked bootloader to flash.

You don't have to unlock the boot loader to flash a signed (original) file. You only need to be unlocked to flash non stock images.

Good to know, I haven't owned a Samsung for a while. But I assume even flashing stock image will up the flash counter?

Not at all. Only flashing non-signed images will up the flash counter. Verizon, for example, has a tool called the "Verizon Phone Support Tool". Basically, you attach it to your device and it will restore or repair your phone. It uses a modified version of Kies to flash a signed/stock image.
