Can't format or use a 16gb sd card on my galaxy?

I am trying to add an Sd card 16gb to my galaxy but when I have placed the card in it's slot and locked it the storage shows "mount SD card" and pressing the start button flashes "0.00B". Any ideas?

1) The phone can't see that large a card. (It depends on the phone and you haven't told us which one it is.)

2) The card could be defective. Try it in a card reader in a computer.

3) The card slot in the phone could be defective.
