Correct settings for service with Family Mobile on an unlocked HTC One Max?

[AC Question]

I purchased an unlocked HTC One Max phone. I am trying to set up with Family Mobile who is powered by T-Mobile. I have been on the phone with representaives at Family Mobile and they cannot activate my phone. We have gone into setting trying to make changes and the phone is still not working.
