how can my sd card fixed?

my memory card always demand to be formatted.And when formated on my phone it looks healthy but when I try to add files on it, it suddenly gone error. the process always repeated. how can this be fixed?my other sd card can't be read by phones.. it does appear on my phone.

Where did you purchase this card? Many cards on eBay are counterfeit cards sold by dishonest sellers. They label the card as being much larger in capacity than it really is.

Try SD Insight to check that the card is not counterfeit.
