The last few days I realized something about my note 5. When I would charge it at 0% it would automatically go up to 16% or 15% and when I use it throughout the day when it hits 20% it goes down so quick like every minute 2%. I let it completely die then charge and same thing so I did a hard reset and it still happened.

Welcome to Android Central! It's probably due to normal battery wear and tear. How old is the phone?

First - NEVER allow the battery to go down to 0%. Never let it get below 40%, in fact. Doing so (bringing it to 0%) kills the battery quickly. If it's going from 0% to 15% almost immediately, it sounds as if the battery has a dendrite (https://www.google.com/search?q=lith...utf-8&oe=utf-8 ) and the battery has to be replaced.
