stock Phone icon vanishes

My stock phone icon in the app dock keeps disappearing. It seems to be totally random-here today, gone tomorrow. Sometimes a reboot brings it back, sometimes not. The icon label is always there, just no icon. If I touch where the icon should be, the phone app starts as normal. It doesn't change how the phone functions, but is a mystifying glitch. Anybody know a cause/solution?

This happened to me once on the Sprint version. I'm on AT&T now and it never happened with this device. Reboot seemed to fix it.

I have the Verizon Note 4 and the same exact thing is happening to me. And I am using the touchwiz launcher, not a third party launcher.

I had the same issue but with the playstore icon. Reboot did not fix it. I then pulled the battery for about 1 minute. That was 2 weeks ago no issues since.
