Stock Music App On The Lock Screen

Somethings up with the stock Samsung music app. For some reason every time my phone rings, or just randomly, the music controls will show up on my lock screen as just a blank black box with a play/pause and track forward buttons. I have tried everything to eliminate this such as:long press to remove (which does nothing, it only elongates the app) opening the app and going to menu > end, clearing the cahe and data in application management, adding it then removing it to lock screen widgets, turning lock screen widgets on then off, restarting the phone, and doing a hard reset. I am completely out of ideas and would like to not have to root my phone just to delete the app itself. I am going crazy trying to figure this out and have searched high and low for answers, even having called Samsung, they give me no support except recommending a hard reset.
Can anyone help me out here? I know I am not the only one having this issue.

it has not been added to my lock screen by choice. It just shows up every once in a while for no reason.
