Lanyard cases for active use?

I live in the Pacific Northwest where I hike and climb a lot. I encounter a lot of terrain types, rocky, snow, dirt, etc.

I need an inexpensive case (not an Otterbox) which has two attachment points for a lanyard and which I can mount to a retractable tether on my backpack so I can grab my phone at a moments notice and take readings, check heart rate, gps, photos, etc. Most cases I've seen have only one attachment point on the corner.

I currently use an armband but they are a pain in the **** to handle when I need to access the phone and the attachment is weak at best. I almost lost a phone on the mountain using this and it was only chance that I was able to recover it.

Would appreciate any pointers!
