It's a good sign that it still shows up in Safe Mode. There are some situations where malware can be installed to the system root, which wouldn't go away in Safe Mode -- but that would probably require some of the risky behavior I mentioned before (i.e., sideloading apk files or rooting the phone and installing unsavory apps).

Try going to Settings>Apps​, then tap Menu>Reset All App Preferences. Now see if the notification shows up again.
 
The closest thing I can find in apps is "android accessibility suite" and "android auto" and "android system webview" all of which are said to have been downloaded from google play store. Also, I'm pretty sure the verizon stuff has been installed since the phone was new. I "bought" it directly from verizon. Also, there are multiple verizon apps that still appear in safe mode so they are "native" to the phone.

When I click on Digital Secure I get a similar message that says "Turn on your security features to get the ultimate protection for your device." and when I click on My Verizon Guided Navigation I get a message that says "When Assistive touch service is active, your device provides access to to read the elements present on the screen. This is used by My Verizon application to provide assistive tutorial to navigate". These have always been off
 
If you dont need them see if can disable or Uninstall or at very least leave them off in accessibility, what about the other service switch access?

1. I tried resetting app preferences (in safe mode) and it seem to have no affect
2. yes, it only shows up on that menu
3. I have never rooted. Just maybe a factory reset a long time ago
4. I do remember when I downloaded sports betting app that it may have required sideloading or giving it special permission or something. I also see those sports betting app files in my download history and they show up as apk files. I still do have/use/update the app regularly.

Hmm, betting/gambling apps always raise suspicions for me. Did you install it from the Play Store?

Not sure initially, but I do see that it is now in the Google Play Store. It is a legal popular betting app, but I think I am going it give it the axe.
 
I did delete the sports app and re-started phone. It did no good. I think I am going to do a factory reset and then try to delete everything Verizon and Google related except Google play store.

What if i did a factory reset, would that get rid of malware installed to the system root?
 
Factory reset usually won't get rid of malware installed to the system root -- that would require reflashing the firmware. But it's still worth trying.
