App for recording information in categories?

Hi,

I'm looking for an app that will help me keep records. I have a health issue for which I need to note down certain details as and when trigger events happen. The app would allow me to specify categories and then to input data for each of them whenever a trigger event happens. So... I guess it would be customizable allowing me to input what the categories are (on the X axis, if it were a spreadsheet) and then subsequently allow me to input the data (down the Y axis) which would be recorded for safe keeping.

So... any ideas? Would this be difficult for me to code myself (having never made an app before).
