Custom ring tones working intermitently

Nexus S 4G (Sprint) running ICS - in last couple of weeks, my custom ring tones only work sometimes. For a given person, I might get the custom tone on 1 out of 3 calls, getting the default tone the rest of the time.

Any ideas, or suggestions?

Yep, same thing here, is irritating when you set ringtones and they only work sometimes...
