Google search widget issue

So my issue is I have the OK google turned off globally. All settings are off.

However last night my search widget started displaying OK google and listening for me to say that and bring up the Google search. Any idea why it's doing this?

I attached a screen shot and you can see where it says ok google in the search bar. It shouldn't say that nor should it be turned on. Not sure why it's doing this any clues??

Settings -> Google -> Search & now -> Voice -> "Ok Google" detection. Please confirm that "From the Google app" is disabled.

And, if it is, reboot your device (what device, by the way?) and see if the problem persists.
 
Samsung s6 edge. They are all turned off. Tried rebooting. Cleared cache. I've done all the typical stuff.

Yes on the home screen where the search widget is at the top it will. I've had that search widget there forever and I used to have to click on it to search. Yesterday the "OK google" text appeared and now it listens for the command. I'm stumped
