After trial, error and even bouncing ideas off AT&T business support I have come to the following being as good as it gets for the time being.
Background info under default conditions:
While Gear and Galaxy are connected via Bluetooth only the Bluetooth radio is active
While Gear and Galaxy are NOT connected via Bluetooth but remote connectivity succeeds:
My recommendation:

For now this is what I am sticking with even after testing Google Voice and Skype forwarding options and voice mail. This just feels proper and native.
There may be subtle errors above as this is after the fact but my intent is simply to help others out.

While Gear and Galaxy are connected via Bluetooth only the Bluetooth radio is active
- Gear 3G radio unreachable to network (Gear is a essentially Bluetooth headset)
- Calls destined to Galaxy phone number ring on both devices at the same time
- Calls destined to Gear phone number are forwarded to Gear voice mail (likely not yet configured)
While Gear and Galaxy are NOT connected via Bluetooth but remote connectivity succeeds:
- Gear 3G radio becomes reachable to the network (Wi-Fi may be used for data if available)
- After 60 seconds of remote connectivity establishment phone issues conditional forwarding commands to AT&T with a 20 second timeout
- Calls destined to Galaxy phone number ring out for 20 seconds then forward to Gear phone number, then ultimately to the Gear number's voice mail
- Calls destined to Gear phone number can be answered on the Gear
My recommendation:
- Call AT&T to have voice mail disabled on the gear line
- Document your Galaxy device's voice mail access phone number
- Disable Bluetooth on Galaxy and issue the following request to the network from your Gear dialer:
- #002# - Clear all forwarding options
- **67*+1VoiceMailAccess*11# - Sends caller to Galaxy voice mail instead of busy signal when call rejected
- **61*+1VoiceMailAccess*11# - Sends caller to Galaxy voice mail when call missed
- **62*+1VoiceMailAccess*11# - Sends caller to Galaxy voice mail in the event flakey connectivity where Galaxy forwarding still enabled despite Gear being unreachable
- Set your Gear number to hidden from Gear call settings
- Set your Gear to use the Galaxy for texting (uses remote connection via data) from Gear messaging settings
- DON'T give anyone your direct Gear phone number and you should be set for a great experience:
- Folks won't wonder if this is your new number as they won't see it
- Voice mail will make it to your phone's voice mail box for AT&T transcription and an alert will be received
- You shouldn't miss any calls
Caveats:
- Ignoring Galaxy forwarded calls with a text reason still uses the Gear number to send SMS (feels like a software bug that should be corrected in time)
- Calls destined to Gear number directly will prompt for a password or voice mail box if Gear is unreachable or the call is not taken (Again, DON'T give this number out)
- Voice mail number option in settings on Gear with AT&T SIM is locked for right now but you can create a contact for one touch dial into your phone 's voice mail box with password and programmed delays
- Calls from your Gear will not show the Galaxy number and AT&T will not set call masking even for wireless business users, this is why you are best to hide it as explained above to avoid callers attempting to use it
