The icons are from http://forum.xda-developers.com/showthread.php?t=2026635
From there you have to unzip them, i used desktop visualizer from the play store, it allows you to set icons in numerous sizes, i did 1x1. I also disabled the dock within nova, that way the icons fit on the bottom.

It does take some time to do, hope it helps....if you have any more questions let me know.

It's called Launcher8... Free in Play Store. The tiles are there by default. It's up to the user to change tile color/transparency/icon style/widgets Also, technically there's only 1 home screen... It's a continuous vertical scroll, rather than pages that have to be "flipped" thru. It just keeps expanding as you add more icons/widgets. Makes getting to things several times faster than flipping pages

I was experimentin with the slanted text couple weeks ago. Nice one man
All id personaly change is use an icon in same colour for apps in middle of dock instead of txt, a lil android or somethin.
Lovely though.... my phone would fly if my Mrs saw it though

The rom is miui, the icon set is called boss, along with the clock, I'm not into analog clocks either....however I think it goes good with the icons.
