You can get a replacement kit from sites like eBay, but I only recommend this if you TRULY know what you're doing. If not, I strooooongly suggest you just take it to a certified repair center and let them do it.

Replacing the screen is not just replacing the screen. You can damage the phone further and go from a shattered glass to a permanently bricked device.

SpookDroid is right. Unless you are skilled, you will need an exceedingly long list of swear words to use while attempting the repair, and nobody has a list as long as that.
