Laptop supports my sd card but my phone does not support

Dear all,

My sd card is being supported by my laptop. I formated it.But when I use it to my tab3 or Samsung A5 or others , it is not being shown to these devices.

Is my sd card out of order? Or is my sd card repairable? If yes , would you please show me the way?

Dear feldon,
I formated my sd card by your mentioned software. I succeed. When I kept it to my phone , it was OK for about some minutes. After that again my phone did not show sd card. It shows "please insert sd card".
What should I do now?

Actually my device shows" your phone does not have SD card inserted "

Is this a known brand or a cheapy SD card off Amazon or somewhere? There are a lot of counterfeit cards out there that say 32GB but only 4GB works and then it fails.
