LG Micro SD Formatting Woes in Android/OSX! Sandisk Showing 200mb instead of 128GB

Since I took my SD out of my Moto G and put it in my new LG G4 it erased everything and has only shown up 200mb. I formatted it using the phone and the same result. I formatted it using my mac in various ways such as FAT, EXFAT, GUID Partition, Masterboot Partition. Same result. I overwrote and formatted it using LG recommended SD formatter 4 on mac. Same result. I managed to get it to show about 200GB by formatting in using my moto G but then I put the card in my G4 and it shows 200mb. Then if I put it back in the moto G it shows 200GB again. I tried using gparted but its just too complicated to bother with installing on mac. And this is coming from having two terrible lexar SDs that corrupted and needed reformatting every few weeks. I actually want to eat my SD card. What can i do?

Welcome to Android Central! This card might be corrupt as well, or it might be counterfeit. Run SD Insight to check its validity.
