I have no idea Ki, I was 8611 and got rejected at first then resent all my stuff and was validated...But it only took 5 days to get my stuff once I was approved...good luck..

Is there any info on the SD card? I was reading about Sandisk Ultra having low random 4K writes scores as compared to the Samsung Evo. Looking on Amazon, there is a solid black class 10 SD card, but wonder about any benchmark testing for it.. I ordered a 32Gig Evo for $14.

I just found out I got denied today. You can't see that I bought a G4 on my T-Mobile receipt. I bought it online and the receipt is 2 pages long. It's impossible to show the order date and that it was a G4 in 1 picture. The IMEI picture I took was the side of my G4 box!! So I'm going to upload two files this time and file it again tonight after work.

It's impossible to show the order date and that it was a G4 in 1 picture. The IMEI picture I took was the side of my G4 box!! I sent mine as one file as well, but it was just approved today (submitted on the 8th). My receipt isn't 2 pages though, as it was bought in store.

I just had to email as well. Mine was also purchased online. They said I didn't submit the IMEI (which I did). Oddly enough, the t-mobile 128gb sd card says it has been approved. It hasn't shipped though.

I also purchased mine online thru tmobile, the email they sent me confirming the purchase was on one page. It should of been the first email you received. It has all the info they are requesting.

I've been approved for the 128gb card for 2 weeks now but it still hasn't shipped.

LG is having to do approvals for every G4 -- regardless of carrier whereas T-Mobile is only having to do their customers. The volume is greater on the LG side.

I know. I just found out mine was denied today. So I'm going to apply again later tonight. I'm not expecting my LG stuff to come until August if I get approved.

I submitted 4 pages combined into 1 pdf. I try to make it as easy as possible for the companies when doing rebates. Some companies will use any excuse (good or bad) to deny you.

Kind weird. I went online to check my status after reading the last few posts and it shows as "Paid". It also shows it has been shipped, but no shipping info other than that. I didn't get any emails regarding approval/shipping (email address is correct) but I guess I'll have my stuff soon.
