my sd card is not mounting on my android phone

Sir, I am having Sony Xperia c 1904 android mobile, and am using transcend 16gb sd card. Recently my memory card is not mounting properly, when I connect sd card, it shows "preparing sd card", then,, it shows "damaged sd card ". But when connect it to the pc through card reader it's working properly.

I don't what's happening, so I tried the common solution chdsk /r like that and I also formatted it. But it was showing the same problem.
I don't know whether problem is on my android sd card slot or problem in sd card. Pls help me to get out this issue

Welcome to Android Central! Do other SD cards work normally? If so, I'd just discard that Transcend. SD cards are pretty inexpensive these days.
