Need help submiting free SD card offer on Verizon

I ordered two Note 7's over the phone from Verizon. They arrived and activated and my wife an I are enjoying them just great. However, I'm having two issues trying to submit the promotion for the free SD cards.

1) What did you submit (if you're on Verizon) to satisfy the requirements? Looking through the website receipts/documents area, I see a bunch of different receipts, customer payment agreements, activation confirmations, etc., but not one of those seems to have all the right info, i.e. Full name, printed date and device. The receipts don't show my name, the payment agreement doesn't say what phone it is, and the activation doesn't show the device, just that one was activated. Can you let me know what you submitted, or if I can upload multiple receipts into the promotions webpage.
EDIT: I'm an *****. The receipt that came in the box had all the info.

2) I tried doing this last night and ran into the above issue so trying again today, however, when I start at the samsung promotions site, I can't enter in my date of birth! I select the month, but it only gives me day options from 1 to 5. I'm not sure if the problem is with my computer or their website. Can someone else just start a new promo submission and see if they have the same issue? You don't need to enter any info, just pick a carrier and on the second screen try to enter a date of birth.

Pardon my stupidity. I've been looking online for a receipt that met the requirements. Didn't think of the one that came in the box with the phone. Duh.

Still having trouble with the website though...

I've been trying to submit mine for the last 45 mins using 3 separate browsers. Every time I try to fill out the online form, I'm unable to select the DAY of my birth. It only displays the numbers 1, 2, 3, 4 & 5. Just attempted to call Customer Support and hung up after waiting 40 minutes. ARGH!
 
That's exactly the problem I have. Tried ms edge, ie, chrome, two computers and my phone. Unless you're born on the first 5 days of the month looks like we can't submit. Grrrr
 
I took a picture of the receipt in the box and it worked fine.
My receipt from store did not list items individually and since I added a case and screen protector it was 964. Sammy turned it down. They did accept the one from the website. I only sent the page that listed phone and price. First page of internet receipt.
 
I just went to the site on my Note 7 and it loads all the dates correctly.....Im using the chrome browser

Well, I can get all the days...but, once you select a month of birth I get restricted to 5 days. Did you choose a month?
 
So I found a workaround, or so I thought. I used an email address that I previously got a Samsung promo with. It skipped all the basic info. Got to the next screen. It asked for purchase date, days only go to 23! I ordered on the 24th! Ugh...
 
Dpham, was yours approved? I finally got it submitted yesterday, used the receipt that came in the box. Just got an email saying "your receipt doesn't show a participating device purchase." I even circled my name, purchase date, and the info for the Note7. What the hell. Did I **** off Samsung somehow?

How did u order ?...I cropped my online receipt and sent them that and got approved....the one they sent me with my phone was more of a shipping receipt ..I ordered in store from att and got it delivered ....
 
I ordered over the phone and had it shipped. I used the receipt that came in the box the phone came in
 
I got two emails. One to accept the agreements, which doesn't show the device, and a shipping/tracking one that doesn't have the order date. In my online account I have documents that look like register receipts but don't show my name, activation confirmations that don't show the phone. The one in the box had it all, and others said they used that with no issue. I'm going to resubmit it again. If it kicks back I'll have to call them and find it what's the problem.
 
So I submitted the exact same receipt that I did the first time. The one from the box. Just got approved. Go figure
