Need to Lock bootloader and return to Verizon

Hope for some help, I rooted and unlocked with stock. My phone took the OTA 2 weeks ago while I was away.
Long story short my bootup screen shows unlock symbol and custom.
Verzson is sending me a new phone due to hardware issues and I need to lock the bootloader.
EZ Unlock? No longer availabe.

Thanks for all the help. Rerooted then used XDA Unroot thread all is fine.

you didnt need a new ph.. you just needed to go to

that would bring yer phone up just as it came outta the box ..well with all the nickle n dime updates then search for an OTA "if ya want it" it will update to 4.1.2
factory.. I've thought about re-rooting but as yet have not seen anything i was comfortable with that rooted the 4.1.2 .. I've already been down brick lane and dont wanna make another trip there.
