Need to send phone back (warrenty) revert back to stock

I have to send my DInc back to VZW, they are exchanging it under warrenty. Right now I am running SkyRaider 2.5.2 with radio version 2.15.00.07.28 I need to know how to get it back to stock so that I can send it back.
