Nexus 4 mystery problem

So my Nexus 4 was rooted stock and running Faux Kernel, nothing out of the ordinary. I've been using it for about 6 months now since I got it the first week of December and there have been no big issues until now. Friday afternoon I tried showing my friend a video in my gallery and it just shower a blank screen with the video timer slider thing, 00:00 pout of 00:00 progress. I closed gallery and tried watching it again and it just ended up with a pop up saying "Can't play video" then got buggy and I couldn't navigate gallery anymore. I turned my phone off and tried again, same problem. I cleared my cache and Dalvik cache thinking that it might fix it but nothing happened. Saturday my phone was taking picture and videos fine, but then I suddenly lost the ability for what seemed like no reason. It gets very buggy when I try to take a picture and crashes when I switch my phone to video mode. I also cannot use apps that involve the camera like Snapchat, taking picture//videos makes it crash, and trying to watch a video just makes a blank screen pop up and leaves the video unwatched. Youtube videos work fine, viewing snapchat photos works fine. It's all I can give you right now, if you need any info just let me know and I'll be glad to help you help me. Thanks in advance!

Edit: I also tried fully formatting the phone and installing a google stock image through fastboot. NO HELP.
 
If you've restored it to the base state and these issues continue, it sounds like a hardware problem actually. I know that seems weird, but I was going to start walking down some troubleshooting steps and after re-reading your original message, all I can come up with is either your recovery ROM image is corrupt, or there's some flaky hardware -presumably somewhere in the camera / imaging area - that is causing the problem.

If its the ROM, you could try loading a different rom, maybe Cyanogen, and see if that fixes the issue or if it continues. If it fixes it, sounds like your recovery image is fried. If it continues, to me that would indicate a hardware issue, and I'd be on the phone with Google asking for an RMA under warranty (after restoring to the base image of course).
