Problems keeping screen dim during handset call

Has anyone had an issue with the screen not staying dim when they rest the phone on their shoulder during a call? It keeps popping up and pressing buttons. Happens on both my left and right shoulder.

Proximity sensor is bad. OR could be covered up. Do you have a case on your phone? screen protector?
