Proximity sensor keeps failing

I installed an app to help test the proximity sensor because when I'm on a call the screen will turn off like it's supposed to but then won't turn on without me pressing a button. The results of the test are that the sensor is active and when I hold my hand over the phone it detects the change. But when I take my hand away it won't detect that it's moved. I did a reboot and factory reset and neither worked. Then I did a battery pull and it worked for a day. Now it's broken again. Any thoughts?
 
Just fid another test and it looks like the proximity sensor works flawlessly in bright light but in the shade it will not detect once I've taken my hand away.
 
Try cleaning it, and remove any cover or protective screen.

I did my testing with no case or screen protector. I going to call samsung tomorrow. Has anyone had to deal with their warranty dept? Is it pretty easy to get a replacement phone?
 
So I called samsung and they had me try to clear the cache partition. When I tried to do that I got an error. They told me to send the phone in for repair. So frustrating
 
I have noticed with mine, if I am holding it in my hand it won't work. When laying flat on a table not holding it it works every time.
