QPST Service Programming - Settings?

I corrupted my Service Programming settings using QPST and can no longer connect to 3G/EVDO... I've tried flashing back to a stock rom and using update profile to reprovision but had no luck.

Can someone please share screenshots of all the QPST service programming tabs or better yet, a backup .QCN file from their settings, so that I can try to restore them on my phone and get data working again?

After flashing back to stock V9 *and* stock V9 recovery, I would suggest you do a ##786# reset, then let hands free activation do its thing. If your NVRAM is corrupted, no amount of Update Profile's will work until it is resolved. ##786# resets most of NVRAM so often it can get by that problem.
