Verizon, made in China.

Film on front, and over camera lense. Nothing on home button. And the film on the back wrapped around all 4 sides protecting the metal frame.

Carved in place by my Note 4.

I just found this, actually saw a thread a couple weeks ago that said "remove the plastic" to some guy who wasn't getting good images. My images were FINE, but then I noticed the little plastic thing and took it off.
there actually WAS NOT any center hole to allow the camera to work normally, it was just shooting through the plastic and didn't really seem to affect quality at all.. ah well, it would have gotten grungy over time.

This was brought up last year with the Note 3. I'm leaving it on this time as it acts as a protector. LOL

Use painters masking tape.
