Scrolling "Roaming Indicator Off" notification

That's where the carrier name should be. So, looks like your carrier is called 'Roaming Indicator Off' :)
On restart, it briefly does say US Cellular there in the upper left notification area before changing to a scrolling "Roaming Indicator Off" message.
 
Remove and re-install SIM?

Or go into mobile network, advanced, and update device configuration?

Removing and reinstalling SIM didn't work. Now I'm on your 2nd method, but what do you mean "update device configuration'? I don't see that option under "advanced"

Try Settings > Network & Internet > Mobile Network > Advanced > Carrier settings > Update Device config
 
"Home only" and "Automatic" - I tried both without success

Check also if you are on LTE only or GSM/Global.
I've searched google for "Roaming indicator off" issue and there seems to be a lot of posts regarding this issue... Sometimes it is incorrect radio mode setting on the phone... LTE Only VS Global or GSM, etc.

There is not a "Update Device Configuration" option. Just "Carrier" and when I go under than, it reads "US Cellular" and "Add Carrier"

When you are on the mobile network screen, when you hit advanced, you should see carrier settings on the bottom of the list.

You can try one more thing ,
Click on carrier then add carrier select back button twice then tap carrier again sim card it will say switching carrier then recheck if it goes away. Also i dont know if you tried safe mode and problem is still there?

I don't have that option on my phone - as in the above screenshot, the bottom-most option is "access point names". I can't scroll further down. That screenshot shows the complete list of options under Mobile Network>advanced
 
I spoke with US Celluar technical support. He's says it's not a message he's seen before, but it is indicating my phone is not currently roaming, which is true. So Maybe it's just something that's there that's supposed to be there, or some bug that might get fixed with a future update. It seems ***-backward, though. And I hope it tells me when I'm roaming. Because THAT I would want to know, I don't really care to know I'm "not roaming" and that my "roaming indicator is off" - I can see that. Just like I wouldn't need to see a scrolling message on the phone that "wifi indicator is off" or bluetooth indicator is off" . So I'll just live with it. it's not a constant thing.

Go into settings > system > Advanced > Reset options > Reset Wifi, mobile & bluetooth. This will force a reset of all connected configurations, then the phone should restart. (If it doesn't restart, restart the phone. Then the phone should re-activate / provision with carrier. (NOTE: This will erase all wifi connections and bluetooth if you have any)....

Just did that. No change
 
US Cellular is a CDMA carrier. It looks like something wasn't properly supported between the carrier and Google. Complaint to both and hope they can fix it. I don't think there is anything on your side can do.
