Does anyone use Verizon messages plus? It keeps prompting me to fill out 911 info and I fill it out but there's no submit button so literally every text I send I get this pop up. I've used this program for years without any issues. This is new, any ideas?

According to this link, this is only supposed to be an option for tablet calling via Wi-Fi.

Step 3 includes a link to the tablet calling feature as well. I don't have this phone, but my wife is on the Fold 3 and didn't see the tablet calling feature. I'm wondering if there's confusion on what the app is seeing the Fold 4 as.

If nothing else, can you try the use current location option? I would think that is the preferred way anyway based on what I was seeing on the Verizon link. That uses GPS based location info, which I expect updates as you move. Otherwise, a Wi-Fi based 911 call would be always routed to your home dispatch center and not necessarily the one covering your location while traveling.

What's also confusing is the reference to 011, which isn't a valid U.S. emergency number.

I have been using the current location. The save button lights up and I hit save but it doesn't do anything and invariably the whole thing comes back in a day or less. It's so odd. I may have to switch text programs in order to avoid it.

It must be recognizing the Fold 4 as a tablet since occasionally I will open the phone to look at texts

If you enable wifi calling, this must be complete to be on file I believe.
