volume unexpectedly goes up using 4.1.2

When listening to music on Pandora or Rocketplayer, the volume suddenly becomes very loud. Does it repeatefly every 10 minutes or so. Any thoughts?

I was encountering something similar. Sometimes, when making or receiving a call, the earpiece volume would go all the way up. I thought it was being caused by an app, but I could never isolate the culprit, and I finally stopped trying. I happened to do a factory reset to address some battery drain issue some, and my volume problem went away.
