Why does my Note 9 always say "alarm" when it rings or "messages" when I receive them?

Why does my Note 9 always say "alarm" when it rings or "messages" when I receive them?

I already had Bixby and Google Assistant disabled and nothing is switched on in the accessibility settings (screen reader, visibility, hearing, interaction and dexterity, etc).

For the alarm, it is not like my phone reads aloud the alarm time although sometime before, I recall hearing something like "It is 8:30 am." Now I just hear it saying the word "alarm" about one second into the alarm ringtone sounding. Bixby alarm is turned off and "read time and alarm name aloud" in the alarm ringtone setting is also turned off.

I have updated my Samsung Galaxy Note 9 phone to the latest version but could this be a result of some kind of bug?

Try safe mode?
