Why is my battery draining even when it is plugged to charge?

My phone is behaving strangely from three days. I leave it to charge but it doesn't. instead it drains out.

Try using the adaptor that came with your phone.Some adaptors are rated for lower amp output as it would damage the device.
unplug the original or other adaptor and you can see their amps.

If you are not using a third party adaptor.Try turning off all the extras.such as gps, sync and use wifi over 4g/3g.
The biggest factor is usually brightness so try and keep that low.If you have low reception your phone will continually search for better service.If you live or spend a lot of time in such an area it would be advised thaat you turn on airplane mode.
If it is a massive problem until you can get the device repaired you should power it down.
