Why is my Note 5 not fully charging?

I bought a Galaxy Note 5 2 days ago and when I charge it may it be on or off it always stops at 92% and there was one incident that it went to 100% but I left it for 5 minutes and it went down to 92% while it was still charging.

Safe bet is to get it replaced with a new one?

What kind of charger are you using? and have you tried a different charger?
 
That might be an indication of bad cells in your battery, I would error on the side of caution and exchange out the device.
 
I have tried 3 different chargers and usb cables and still got the same result. I guess I have to get it replaced.

I got my phone replaced with a new one and I hope it doesn't repeat that issue. Should I skip the update?

No, that didn't cause your issue. If you ever want to think about using Samsung Pay or keep the security up to date on your phone, you need to update. The only updates for the Note 5 have been advanced messaging (T-Mobile), security updates and Samsung Pay.
 
I live in the Philippines and I got the Duos version and I have attached the screenshot of the update.
