Same goes with me.. i called via their hotline they provided me with some instructions..
1) you need to insert a sim card on your O+ phone with regular load (leave it switched on)
2) the number you provided as a response to your anti-theft security would have to send a particular code via the sent message from the service provider (however i've lost that certain message from the SP mentioning that you have changed simcard with an auto response)
3) you have to go to their service center so that they can reformat your phone (ofcourse with 500 pesos payment for non-warranty phones) you will need to present your receipt of purchase (which i also lost) all i have is the box with the codes provided. tskkk...:'( :-\
