Why is my rear camera blurry and foggy?

I noticed that my rear camera have camera very blurry and foggy but my front came is still working perfectly. It used to be very clear but now I can't even use it. There are some minor scratches on my lens so I tried to fix it by putting toothpaste using cotton buds and wiping it clean using a microfiber. It kind of made it a bit clearer but still nowhere good as it was before. Please help
