why does a Triangle with a exclamation point in it stay on my phone?

I did a factory reset and every since a triangle with a exclamation point in it is always on my phone. It says Google Talk Authentication Failed. what do i need to do to get it off and so my phone will act right again?

I have a similar issue with my HTC M8 with ATT. I have a red triangle on my children's name in messages?? they do not go away , and the phone does not have a way to address what they are?
