Why is my Note 3 battery draining so fast?

I charge my phone all night and in the morning it still says it is on 70-90%, the charger has no problems since i use it only when i'm at home or in school and its never mishandled . I could leave the phone alone for 5 minutes when it is on 100% and when i get back its on 70-80% and i have to keep charging it every 2 to 3 hours and it turns off automatically when it is on 19%. PLEASE HELP!! Also i don't have much apps that make the battery drain since i only have two apps that use about 5%. do i need to replace the battery ????
