black screen freeze and not clickable and sometimes reboots it self

So i have a sm t231 and its rooted, my phone sometimes freezes and shows black screen but the status bar is still there but not moving and the buttons are not clickable except for the power button, when I Long Press it, it shows the device options but not clickable and sometimes it reboot it self, after rebooting, it will work again but it happens too often and im tired of rebooting my phone again and again. Please help
