Why does Dialer keep adding +1 to numbers?

Whenever I try to voice dial or place a call from a website my dialer keeps adding "+1" to the beginning of the number. Consequently, my service cannot connect and I have to go back and physically enter the number or delete the +1 and try again. It might be worth mentioning that voice dialing through a third-party bluetooth device (i.e. car audio or earbuds) does not cause this result.

Is there a tried solution I could pursue?

+1 is the country code for the US. Perhaps the number you are trying to call is not in the US? But then the country code should be the code the other country if that's the case. You can't edit the number you're going to calling before you place the call?
