Why is my Lexar micro SD card unsupported?

For the past couple of week, it was working perfectly, but now it wont format and keeps saying that the device doesn't support the lexar sd card.If it helps, I have Motorola G3.

The card is probably shot - and the Lexar "warranty" is almost useless. Stick to SanDisk cards in the future. (They cost a little more, but they're replacement-guaranteed [which still means that you should back everything on the card up to another device] and they have online tech help.)
